Fisher (2017), described a scenario where a psychologist went to the home of a client who could not attend an appointment of the health center to discuss patients and families end-of-life decision-making sessions due to the patients being diagnosed with a terminal disease. The Psychologist discusses information with the patient in her home, and the Psychologist did not document the visit in the patient records.

The Psychologist not documenting the patient records is a violation of the ethical code of conduct Principle A: Beneficence and Non-maleficence and Principle B: Fidelity and Responsibility (Fisher, 2012). It is the Psychologist responsibility to maintain proper documents and records for those whom they work with as well as maintain records on individual clients that is necessary to provide appropriate assessment, planning, evaluation, and modification to services as needed. Also, Standard 6.01 states one is to create, maintain, store records that are to be used appropriately by Psychologist and others to help service the client (Fisher, 2017).

To resolve the ethical dilemma is to document the sessions in the patient’s files. The suggested strategy of the document the session should be detailed with accurate information that is descriptive enough for others to evaluate (Fisher, 2012). Even if the Psychologist documented the session and the information was not written legibly to be read by others, this could be another dilemma of ethical violations. If one can not read what is written, one cannot make an effective decision on a patient to provide the necessary treatments or services to a client (Fisher, 2017). Therefore, one should take the time to write legibly when documenting information in a patient file.
